What is an Underbite? (Mandibular Prognathism)

An underbite occurs when the lower jaw (mandible) protrudes significantly beyond the upper jaw (maxilla). This skeletal misalignment causes the lower front teeth to sit in front of the upper teeth when the mouth is closed.

While mild dental crowding can sometimes be “masked” with braces or clear aligners, a skeletal underbite is a structural bone issue. In these cases, orthodontics alone cannot reach the root of the problem. Orthognathic surgery is required to physically move the jawbone into its scientifically correct position, ensuring your teeth meet perfectly and your facial profile is balanced.

Why Surgery is Necessary: Beyond the Mirror

A skeletal underbite isn’t just a cosmetic concern; it is a functional hurdle that affects your daily quality of life. If left untreated, the constant mechanical strain of a misaligned jaw can lead to long-term health complications:

  • Accelerated Tooth Wear: Because the teeth do not meet correctly, the enamel is forced to bear unusual pressure, leading to chipping, “flat” teeth, and premature decay.
  • Difficulty Chewing (Mastication): Patients often struggle to bite into common foods like apples, sandwiches, or pizza, as the front teeth cannot “shear” through food effectively.
  • Speech Impediments: The tongue requires specific contact points to create sounds. An underbite often causes “lisping” or difficulty with “s,” “sh,” and “f” sounds.
  • Chronic Jaw Pain (TMJ): The jaw joint is forced to compensate for the misalignment, often resulting in tension headaches, clicking, and popping in the TMJ.

The Surgical Solution at Love Your Jaws

At our South Miami surgical suite, Dr. Kroum Dimitrov utilizes a combination of hospital-grade expertise and cutting-edge technology to correct underbites with millimeter precision.

Mandibular Osteotomy & Le Fort I

Depending on your unique anatomy, Dr. Dimitrov may perform a Mandibular Osteotomy to move the lower jaw backward. In many cases, we also perform a Le Fort I Osteotomy to move the upper jaw forward. This “double jaw” approach often produces the most natural and aesthetically pleasing profile.

The 3D Advantage

We don’t leave your results to chance. Using 3D Virtual Surgical Planning (VSP), we create a digital “twin” of your skull.

  1. Digital Simulation: Dr. Dimitrov performs the entire surgery on a computer before your appointment.
  2. Guided Precision: We use custom surgical splints to ensure the bone is moved exactly where the digital plan dictates.
  3. Predictable Esthetics: We can show you a preview of your new profile during your consultation.
